    Area of Science:

    • Neurology
    • Neuroscience
    • Pathology

    Background:

    • Multiple sclerosis (MS) is a chronic inflammatory demyelinating disease of the central nervous system.
    • Hypertrophic demyelinating neuropathy is a rare condition characterized by nerve enlargement and demyelination.
    • Co-occurrence of MS and hypertrophic demyelinating neuropathy is exceptionally rare, with limited documentation, primarily from autopsy studies.

    Observation:

    • A patient presented with clinical features consistent with classical multiple sclerosis.
    • The patient also exhibited areflexia and sensory abnormalities in a stocking distribution, suggesting peripheral neuropathy.
    • Nerve conduction studies revealed significant abnormalities indicative of a demyelinating neuropathy.

    Findings:

    • Sural nerve biopsy confirmed the presence of hypertrophic demyelinating neuropathy.
    • This case provides definitive in-vivo evidence of the co-existence of MS and hypertrophic demyelinating neuropathy.
    • The diagnostic findings underscore the importance of comprehensive neurological evaluation.

    Implications:

    • This unique case expands the understanding of the spectrum of neurological presentations in MS.
    • It suggests that hypertrophic demyelinating neuropathy should be considered in MS patients with peripheral neuropathy symptoms.
    • Further research may elucidate shared pathophysiological mechanisms or diagnostic approaches for these combined conditions.
