    Summary

    Patients on chronic haemodialysis show significantly elevated plasma levels of beta-endorphin (beta-EP) and beta-lipotrophin (beta-LPH). Haemodialysis treatment effectively reduced these elevated peptide concentrations in uraemic patients.

    Background:

    • Uraemia, a condition associated with chronic kidney disease, can lead to hormonal imbalances.
    • Beta-endorphin (beta-EP) and beta-lipotrophin (beta-LPH) are key peptides involved in stress response and pain modulation.
    • Understanding peptide levels in uraemic patients is crucial for managing associated physiological disturbances.

    Purpose of the Study:

    • To quantify plasma immunoreactive beta-endorphin (IR beta-EP) and beta-lipotrophin (IR beta-LPH) in adult patients undergoing chronic haemodialysis.
    • To investigate the impact of haemodialysis on these peptide levels.

    Main Methods:

    • Plasma samples from 15 adult uraemic patients on chronic haemodialysis were analyzed.
    • Gel permeation chromatography (Sephadex G-50) and sequential immune-affinity chromatography were employed for peptide separation.
    • Radioimmunoassay (RIA) was used to measure the concentrations of IR beta-EP and IR beta-LPH.

    Main Results:

    • Plasma IR beta-EP concentrations were markedly increased in uraemic patients compared to normal subjects (64.4 +/- 13.7 fmol/ml vs. 2.3 +/- 0.2 fmol/ml).
    • Plasma IR beta-LPH concentrations were also significantly elevated in uraemic patients compared to normal subjects (55.7 +/- 13.2 fmol/ml vs. 6.1 +/- 0.8 fmol/ml).
    • Haemodialysis resulted in a reduction of both plasma IR beta-EP and IR beta-LPH levels compared to pre-dialysis concentrations.

    Conclusions:

    • Adult patients with uraemia on chronic haemodialysis exhibit substantially elevated plasma levels of beta-endorphin and beta-lipotrophin.
    • Haemodialysis appears to be effective in lowering these elevated peptide levels.
    • These findings highlight potential hormonal dysregulation in uraemia and the therapeutic effect of dialysis.
