    A rare case of thymoma and insulinoma co-occurrence led to severe hypoglycemia and death. Thymoma is frequently associated with other malignancies, particularly hematopoietic ones.

    Background:

    • Thymoma, a tumor of the thymus, can be associated with various paraneoplastic syndromes.
    • Insulinoma, a pancreatic neuroendocrine tumor, causes hypoglycemia due to excess insulin secretion.

    Observation:

    • Autopsy of a 44-year-old male revealed invasive thymoma and pancreatic insulinoma.
    • The patient presented with severe hypoglycemia (26 mg/dl).

    Findings:

    • Immunohistochemical analysis confirmed the diagnosis of insulinoma.
    • Statistical analysis indicates thymoma is a frequent associated malignancy (9.3% of cases), with a higher incidence of hematopoietic malignancies compared to other cancers.

    Implications:

    • This case highlights the importance of considering dual malignancies in unexplained hypoglycemia.
    • Understanding the association between thymoma and other cancers can improve diagnostic and treatment strategies.
