    Summary

    In computed tomographic (CT) scans, higher bile density than liver density was observed in 8 of 620 gallbladder examinations. Four patients with this finding had hepatocellular carcinoma, suggesting a potential link.

    Background:

    • Computed tomography (CT) is a key imaging modality for evaluating the hepatobiliary system.
    • Bile density on CT typically appears lower than or equal to liver parenchyma density.
    • Abnormalities in bile composition or flow can alter its imaging characteristics.

    Observation:

    • Eight out of 620 gallbladder CT examinations revealed bile density exceeding that of the liver.
    • Serial CT studies showed variable changes in bile density in three patients (increase) and one patient (decrease).

    Findings:

    • A significant proportion (four of the eight) of patients with increased bile density also had a diagnosis of hepatocellular carcinoma.
    • This observation suggests a potential association between altered bile density and the presence of liver cancer.
