    Background:

    • Soft tissue sarcoma is a challenging malignancy, often presenting as inoperable or metastatic disease.
    • Primary chemotherapy is explored as a strategy to downstage tumors for surgical resection.

    Observation:

    • Fourteen adult patients with inoperable soft tissue sarcoma received neoadjuvant chemotherapy.
    • Two regimens, CYVADIC (cyclophosphamide, vincristine, doxorubicin, dacarbazine) and DECAV (dacarbazine, cyclophosphamide, cisplatin, doxorubicin, vindesine), were administered.
    • Objective responses (complete remission or >50% regression) and stabilization were observed in 11 patients.

    Findings:

    • Seven patients (50%) became operable after 2-6 chemotherapy courses.
    • Six of these patients underwent successful complete tumor resection.
