    Summary

    Subacute polyneuropathy associated with hematologic malignancies can present with demyelination. Prednisone treatment showed initial efficacy, suggesting an immune-mediated process in these neurological disorders.

    Background:

    • Subacute polyneuropathy can be an early indicator of underlying hematologic malignancies.
    • Understanding the pathogenesis of neuropathy in these conditions is crucial for effective treatment.

    Observation:

    • Two patients presented with subacute polyneuropathy responsive to prednisone.
    • One patient was diagnosed with histiocytic lymphoma, the other with chronic lymphocytic leukemia.

    Findings:

    • Sural nerve biopsies revealed extensive segmental demyelination in both patients.
    • Histopathology showed macrophage invasion of axons in the lymphoma patient and lymphocytic infiltration in the leukemia patient.
    • Demyelination mechanisms included cell-mediated processes and potential mechanical compression by leukemic cells.

    Implications:

    • Polyneuropathy can be a paraneoplastic manifestation of hematologic cancers.
    • Immune-mediated demyelination is a likely mechanism, responsive to immunosuppressive therapy.
    • Further research is needed to elucidate the precise pathogenic pathways and optimize therapeutic strategies.
