Summary

This case study reports relapsing polyneuritis, a nerve disorder, in a woman with cirrhosis and a splenorenal shunt. The findings suggest toxic metabolites, possibly from portosystemic encephalopathy, may cause nerve damage.

Background:

Reports a case of relapsing polyneuritis in a woman diagnosed with liver cirrhosis.
The patient had undergone a surgical splenorenal shunt procedure.

Observation:

Ultrastructural analysis of nerve biopsy revealed segmental demyelination and myelin breakdown.
Schwann cells showed invasion by mononuclear cell processes.

Findings:

Histological changes are characteristic of Guillain-Barré syndrome.
Alcohol and diabetes were ruled out as causative factors for the observed nerve damage.

Implications:

Suggests a potential role for unidentified toxic metabolites in nerve damage.
Discusses the possible link between these metabolites and portosystemic encephalopathy in cirrhotic patients.
