    Summary

    Hepatocellular carcinoma (HCC) rarely causes obstructive jaundice. This autopsy revealed HCC with bile duct metastasis completely blocking bile flow, a rare but serious complication. Review of 85 cases is included.

    Background:

    • Hepatocellular carcinoma (HCC) is a primary liver cancer.
    • Chronic liver disease is a major risk factor for HCC.
    • Extrahepatic biliary obstruction is an uncommon presentation of HCC.

    Observation:

    • An autopsy was performed on a 68-year-old male with a history of chronic liver disease.
    • The patient presented with marked jaundice and died from gastrointestinal hemorrhage.
    • Autopsy revealed HCC with liver cirrhosis and metastatic growths in the common bile duct and cystic duct.

    Findings:

    • The metastatic growths completely obstructed the bile duct lumen, causing obstructive jaundice.
    • This represents a rare complication of hepatocellular carcinoma.
    • The study reviewed 85 additional cases of HCC with extrahepatic biliary obstruction.

    Implications:

    • Understanding rare presentations of HCC is crucial for diagnosis and management.
    • Bile duct obstruction in HCC patients requires prompt investigation.
    • This case highlights the diverse clinical manifestations of advanced liver cancer.
