    Background:

    • Pathologic differentiation between post-injection changes and neoplasms can be challenging.

    Observation:

    • Pathologic findings after silica injection can resemble malignant fibrous histiocytoma.
    • CT imaging may show a partially calcified inguinal soft tissue mass without bone destruction.

    Findings:

    • The appearance of silica injection sequelae can be indistinguishable from malignant fibrous histiocytoma.
    • Partially calcified inguinal masses in older patients warrant consideration of this entity.

    Implications:

    • A history of silica injection is vital for correct pathologic interpretation.
    • Awareness of this entity aids in appropriate clinical management of inguinal masses.
