    Background:

    • Iron deficiency anemia (IDA) is a common condition with various potential causes.
    • Small intestine diverticulosis is a less common condition, often asymptomatic.

    Observation:

    • Three cases of IDA were investigated.
    • One patient had a colo-duodenal fistula; the other two showed small intestine diverticulosis on barium studies after negative upper and lower GI evaluations.

    Findings:

    • Small intestine diverticulosis was identified as the likely cause of IDA in two patients.
    • Chronic blood loss from these diverticula is the proposed mechanism for anemia.

    Implications:

    • Small intestine diverticulosis should be considered in the differential diagnosis of IDA, particularly when other causes are ruled out.
    • Further investigation of the small bowel may be warranted in select IDA patients.
