    Background:

    • Non-Hodgkin lymphoma can present with rare endobronchial localization.
    • Immunoblastic sarcoma, a subtype of lymphoma, is associated with immunological disorders and carries a poor prognosis.

    Observation:

    • A 48-year-old woman presented with atelectasis and fever, indicative of endobronchial disease.
    • Bronchoscopy revealed an endobronchial mass, diagnosed as immunoblastic lymphosarcoma via biopsy.

    Findings:

    • The patient had selective total IgA deficiency and a family history of malignancies.
    • Surgical resection and chemotherapy were ineffective, leading to a fatal outcome within 7 months.

    Implications:

    • This case underscores the diagnostic challenges and aggressive nature of endobronchial non-Hodgkin lymphoma.
