    Summary

    Pipecuronium bromide is a new steroid muscle relaxant, found to be 20% more potent than pancuronium. Both agents offer similar recovery and reversibility with no observed side effects from pipecuronium.

    Background:

    • Pancuronium is a widely used steroid muscle relaxant.
    • New muscle relaxants are continuously being developed to improve anesthetic safety and efficacy.

    Purpose of the Study:

    • To compare the neuromuscular and circulatory effects of pipecuronium bromide (Arduan R) with pancuronium during balanced anesthesia.

    Main Methods:

    • Two groups of patients were studied: 18 receiving pipecuronium and 20 receiving pancuronium.
    • Neuromuscular and circulatory effects were monitored during balanced anesthesia.

    Main Results:

    • Pipecuronium was approximately 20% more potent than pancuronium.
    • The 95% blocking doses were 0.059 mg/kg for pipecuronium and 0.075 mg/kg for pancuronium.
    • Both drugs provided equal intubating conditions and relaxation; onset, duration, recovery, and reversibility were similar. No cardiovascular or other side effects were noted with pipecuronium.

    Conclusions:

    • Pipecuronium bromide is a potent muscle relaxant with a safety profile comparable to pancuronium.
    • Pipecuronium bromide offers a viable alternative to pancuronium in balanced anesthesia.
