    Summary

    Dapsone (4,4'-diaminodiphenylsulfone) can cause neuropathy even after long-term use at low doses. Identifying slow acetylators and HLA types may help predict patients at risk for dapsone-induced neurotoxicity.

    Background:

    • Dapsone (4,4'-diaminodiphenylsulfone) is a common treatment for dermatitis herpetiformis.
    • Neuropathy is a known, though infrequent, adverse effect of dapsone therapy.

    Observation:

    • A patient developed severe motor and minor sensory neuropathy after 16 years of low-dose (100 mg/day) dapsone treatment.
    • The neuropathy presented as an axonal neuropathy affecting the extremities.
    • The patient was identified as a slow acetylator of dapsone.

    Findings:

    • This case is atypical due to the prolonged duration of dapsone use and low dosage preceding neuropathy.
    • Typical dapsone-induced neuropathy usually occurs within five years at higher doses (≥300 mg/day).
    • Complete recovery is generally observed upon dapsone dose reduction or withdrawal.

    Implications:

    • Suggests that prolonged dapsone use, even at low doses, can lead to neurotoxicity.
    • Recommends HLA typing and acetylation profiling for new dapsone-induced neuropathy cases to identify high-risk individuals.
    • Highlights the need for increased vigilance regarding potential neurotoxicity in patients on long-term dapsone therapy.
