    • Rectal mucosa histology was abnormal in 63.0% of infected men versus 7.4% of controls (p < 0.001).
    • Serum antibodies reactive to E. histolytica were not detected in any patient.

    Conclusions:

    • Entamoeba histolytica infection in homosexual men is associated with prolonged gastrointestinal symptoms and rectal mucosal abnormalities.
    • Histopathological findings are more sensitive indicators than serological tests in this cohort.
    • Further research into the host-parasite relationship and diagnostic strategies is warranted.
