    Background:

    • Thalamic hemorrhage is an underrecognized cause of aphasia.
    • Diagnostic challenges historically limited documentation of smaller thalamic hemorrhages.

    Observation:

    • Presents four new cases of thalamic hemorrhage presenting with acute aphasia.
    • Patients exhibited supranuclear paralysis of upward gaze, right hemisensory deficits, and mild right hemiparesis.
    • Symptoms included acute aphasia, gaze palsy, and sensory/motor deficits.

    Findings:

    • Ventriculostomy drainage led to rapid improvement in gaze palsy in three patients.
    • Two patients required permanent ventricular shunts.
    • One-year follow-up revealed varied speech recovery, with two patients fully recovered and two remaining severely aphasic.

    Implications:

    • These findings challenge current models of thalamic function in speech.
    • Highlights the thalamus as a critical, yet often overlooked, area for speech processing.
    • Suggests potential therapeutic targets for thalamic hemorrhage-induced aphasia.
