    Background:

    • Postoperative bacterial endophthalmitis is a severe intraocular infection that can lead to vision loss.
    • Pain has traditionally been considered a primary early warning symptom of endophthalmitis.

    Observation:

    • Three cases of culture-proven bacterial endophthalmitis were identified.
    • These cases presented without the typical symptom of ocular pain.

    Findings:

    • Abnormal physical findings were key indicators for diagnosing endophthalmitis in these patients.
    • Microbiologic evaluation and prompt therapy were initiated based on physical signs.

    Implications:

    • Relying solely on patient-reported pain is insufficient for early endophthalmitis detection.
    • Ophthalmologists must perform frequent postoperative ocular examinations to identify subtle signs of infection.
