    Summary

    Intrapleural tetracycline (TCN) effectively prevents malignant pleural effusion recurrence. Its efficacy as a sclerosing agent is pH-independent, outperforming chest tube drainage alone.

    Background:

    • Malignant pleural effusions are a common complication of various cancers.
    • Tetracycline (TCN) instillation is a recognized treatment for preventing effusion recurrence.
    • The role of TCN solution pH in its efficacy remains unclear.

    Purpose of the Study:

    • To investigate the efficacy of intrapleural tetracycline in preventing malignant pleural effusion recurrence.
    • To determine if the acidic pH of tetracycline solution contributes to its sclerosing effect.

    Main Methods:

    • A double-blind, randomized trial involving 30 patients with malignant pleural effusions.
    • Patients received either intrapleural tetracycline (500 mg) or a control solution with similar pH and appearance.
    • All patients underwent chest tube drainage.

    Main Results:

    • 24 out of 30 patients were evaluable for effusion reaccumulation.
    • 9 of 13 patients treated with tetracycline showed no fluid reaccumulation.
    • Only 1 of 9 patients in the control group had no reaccumulation (P < 0.05).

    Conclusions:

    • Intrapleural tetracycline is more effective than chest tube drainage alone for controlling malignant pleural effusions.
    • The sclerosing efficacy of tetracycline is not dependent on its acidic pH.
    • Intrapleural tetracycline offers a viable therapeutic option for managing malignant pleural effusions.
