    Summary

    Hemodialysis showed no significant therapeutic effect for schizophrenia patients. Improvements observed in both active and sham dialysis groups suggest transient effects unrelated to blood clearance.

    Background:

    • Schizophrenia is a complex mental disorder with limited treatment options.
    • Hemodialysis, a treatment for kidney failure, was explored for potential therapeutic effects in schizophrenia.

    Purpose of the Study:

    • To evaluate the therapeutic efficacy of hemodialysis in patients with schizophrenia.
    • To determine if blood clearance via dialysis contributes to clinical improvement in schizophrenia.

    Main Methods:

    • A randomized controlled trial involving 19 schizophrenic patients.
    • Patients were assigned to either active hemodialysis (n=10) or sham dialysis (n=9) for one month.
    • Clinical assessments were performed before and after treatment by a blinded investigator using four rating scales.

    Main Results:

    • Clinical improvement was observed in 50% of patients receiving active hemodialysis and 33% of those receiving sham dialysis.
    • No statistically significant difference in the magnitude of improvement was found between the active and sham dialysis groups.
    • Subsequent treatment periods with active or sham dialysis did not yield significant differences in improvement.

    Conclusions:

    • The observed transient improvements in some schizophrenic patients undergoing hemodialysis do not appear to be attributable to the blood-clearing effects of dialysis.
    • Further research is needed to understand the mechanisms behind transient symptom fluctuations in schizophrenia.
