    Summary

    Evoked potential tests like BAEP and VEP can identify subclinical lesions in multiple sclerosis (MS) patients, aiding diagnosis. Combining these tests with SEP and BR improves detection of silent lesions, especially in uncertain cases.

    Background:

    • Multiple sclerosis (MS) diagnosis can be challenging, particularly in early or uncertain stages.
    • Evoked potentials offer a non-invasive method to assess neurological function in MS.
    • Previous studies have explored various evoked potential techniques in MS with varying success.

    Purpose of the Study:

    • To evaluate the diagnostic utility of multiple evoked potential tests in patients with multiple sclerosis (MS).
    • To determine the sensitivity of Brainstem Auditory Evoked Potentials (BAEP), Visual Evoked Potentials (VEP), Somatosensory Evoked Potentials (SEP), and Blink Reflex (BR) in detecting subclinical lesions.
    • To assess the value of combining different evoked potential tests for improved diagnostic accuracy in MS.

    • Recordings of BAEP, VEP, SEP, and BR were obtained from 160 MS patients and control subjects.
    • Stimulation methods included auditory, pattern-reversal visual, and somatosensory stimuli, along with electrical stimulation for BR.

    • High incidences of abnormalities were observed in definite MS patients.
    • VEP and BAEP combined detected subclinical lesions in 71% of patients with suspected or possible MS.
    • Adding SEP and BR increased subclinical lesion detection to 74%; VEP was highly sensitive for optic neuritis.
    • SEP and BR showed lower sensitivity for detecting silent lesions compared to VEP and BAEP.

    Conclusions:

    • Evoked potential tests, while not specific to MS, provide valuable diagnostic information, especially in clinically uncertain cases.
    • The combination of VEP and BAEP is recommended, supplemented by SEP in patients without spinal signs.
    • These non-invasive tests can obviate the need for more invasive examinations in diagnosing MS.
