    Summary

    Lichen planus, a skin condition, frequently co-occurs with primary biliary cirrhosis. D-penicillamine treatment may trigger lichen planus, suggesting a graft-versus-host link in primary biliary cirrhosis.

    Background:

    • Primary biliary cirrhosis (PBC) is a chronic liver disease.
    • Lichen planus (LP) is an inflammatory condition affecting skin and mucous membranes.
    • D-penicillamine is a medication used to treat various conditions, including PBC.

    Purpose of the Study:

    • To investigate the association between lichen planus and primary biliary cirrhosis.
    • To evaluate the role of D-penicillamine in the development or exacerbation of lichen planus in patients with PBC.

    Main Methods:

    • Retrospective analysis of 24 patients with both lichen planus and primary biliary cirrhosis.
    • Observation of the timing of lichen planus onset in relation to D-penicillamine therapy.

    Main Results:

    • Lichen planus developed in 17 out of 24 patients during D-penicillamine treatment.
    • In 7 patients, lichen planus occurred independently of D-penicillamine.
    • Three patients with pre-existing lichen planus experienced relapse or worsening after D-penicillamine treatment.

    Conclusions:

    • The findings support a graft-versus-host disease (GVHD) mechanism in primary biliary cirrhosis.
    • Lichen planus in PBC patients, especially when associated with D-penicillamine, is indicative of potential GVHD.
    • Pre-existing lichen planus is a relative contraindication for D-penicillamine use in primary biliary cirrhosis patients.
