    Summary

    Primary membranous glomerulopathy (MGN) shows significantly greater glomerular basement membrane (GBM) involvement by immune deposits compared to secondary MGN. This quantitative difference may reflect distinct immune complex types or deposition patterns in these kidney diseases.

    Background:

    • Primary and secondary membranous glomerulopathy (MGN) are often indistinguishable using light microscopy and immunofluorescence.
    • Anecdotal observations suggest potential differences in subepithelial immune complex deposit distribution between primary and secondary MGN.

    Purpose of the Study:

    • To quantitatively compare the extent of immune complex deposits on the glomerular basement membrane (GBM) in primary versus secondary MGN.
    • To investigate if electron microscopy can differentiate between primary and secondary MGN based on deposit distribution.

    Main Methods:

    • Electron microscopy was used to examine kidney biopsies from 23 patients with primary MGN and 22 with secondary MGN.
    • The percentage of GBM occupied by immune complex deposits was quantitated for each case.

    Main Results:

    • The mean GBM involvement was significantly higher in primary MGN (49% +/- 15%) compared to secondary MGN (16% +/- 15%) (P < 0.05).
    • Excluding advanced disease (Churg Class IV), this difference remained significant (48% +/- 15% vs. 12% +/- 7%, P < 0.01).

    Conclusions:

    • Quantitative differences in GBM immune complex deposit involvement distinguish primary from secondary MGN.
    • These findings suggest potentially different underlying mechanisms, such as distinct antigen-antibody complexes or deposition patterns, in the pathogenesis of primary and secondary MGN.
