    Summary

    Epikeratophakia surgery is safe for patients with aphakia and endothelial damage. This corneal graft procedure shows no adverse effects like edema, even with pre-existing low endothelial cell counts.

    Background:

    • Assessing the safety and efficacy of epikeratophakia in patients with compromised corneal endothelium.
    • Evaluating post-operative outcomes in aphakic eyes following epikeratophakia.

    Observation:

    • Specular microscopy was used to examine the corneal endothelium in eight aphakic patients post-epikeratophakia.
    • Corneal thickness increased significantly after the onlay graft procedure (0.52 mm to 0.88 mm).
    • Pre-operative endothelial cell counts (1,450/mm²) were low and did not significantly change post-operatively (1,438/mm²).

    Findings:

    • No corneal edema was observed despite increased corneal thickness and low endothelial cell density.
    • The corneal endothelium demonstrated tolerance to the epikeratophakia procedure.

    Implications:

    • Epikeratophakia is a safe surgical option for eyes with pre-existing endothelial trauma.
    • The procedure is well-tolerated, suggesting potential for its use in complex ophthalmic cases.
