Prophylactic antibiotics in elective colorectal surgery.

    Summary

Adding parenteral cefazolin or ticarcillin significantly reduced septic complications, particularly wound infections, in patients undergoing elective colorectal surgery. This contrasts with oral antibiotics alone, highlighting the benefit of combined parenteral and oral antibiotic therapy.

    Area of Science:

    • Surgical Oncology
    • Infectious Diseases
    • Pharmacology

    Background:

    • Postoperative septic complications are a significant concern in elective colorectal surgery.
    • Oral antibiotics like erythromycin and neomycin are used, but their efficacy alone in preventing complications is debated.

    Purpose of the Study:

    • To evaluate the impact of adding parenteral antibiotics (cefazolin or ticarcillin) to oral erythromycin and neomycin on reducing postoperative septic complications in elective colorectal operations.

    Main Methods:

    • A randomized, double-blind, controlled trial was conducted with three patient groups.
    • All patients received mechanical bowel preparation and oral erythromycin/neomycin.
    • Groups received either intravenous saline placebo, cefazolin, or ticarcillin.

    Related Experiment Videos

    Main Results:

    • Septic complications occurred in 35% of the placebo group, 7% of the cefazolin group, and 5% of the ticarcillin group.
    • Wound infections were substantially reduced with cefazolin (4.7%) and ticarcillin (2.3%) compared to placebo (29%).

    Conclusions:

    • Parenteral cefazolin or ticarcillin, when added to oral antibiotics and bowel preparation, significantly decreases wound infections and septic complications in elective colorectal surgery.
