    Summary

Diaphragmatic rupture from blunt trauma in children is increasing. While isolated cases have a good prognosis, severe associated injuries often limit outcomes, and diagnosis is frequently delayed.

    Area of Science:

    • Pediatric Surgery
    • Trauma Surgery
    • Thoracic Surgery

    Context:

    • Blunt traumatic diaphragmatic rupture is a rare but increasingly recognized injury in pediatric patients.
    • This condition presents unique challenges in diagnosis and management compared to adult populations.

    Purpose:

    • To report on four pediatric cases of diaphragmatic rupture due to blunt trauma.
    • To compare these cases with existing literature and highlight key clinical features.
    • To emphasize diagnostic challenges and prognostic factors in pediatric diaphragmatic injuries.

    Summary:

    • Diaphragmatic ruptures predominantly occur on the left side, though right-sided ruptures are seen in one-third of cases.
    • Unlike adults, pediatric diaphragmatic ruptures are infrequently associated with rib fractures.
    • Injuries to abdominal organs like the liver and spleen are common, with two-thirds of patients experiencing organ prolapse into the pleural cavity.
    • Initial diagnosis is often missed, occurring in only half of the reported cases.

    Impact:

    • Highlights the importance of considering diaphragmatic rupture in pediatric blunt trauma, even without rib fractures.
    • Emphasizes the need for improved diagnostic strategies to ensure timely identification of this injury.
    • Informs surgical management and prognostication by detailing common associated injuries and outcomes.
