    This study reports on three plasma cell leukemia patients, two with prior myeloma. All cases showed characteristic enzyme activity in plasma cells, with poor prognoses and short survival times under various treatments.

    Background:

    • Plasma cell leukemia (PCL) is a rare and aggressive hematologic malignancy.
    • Distinguishing PCL from multiple myeloma with plasma cell leukemia is crucial for prognosis and treatment.

    Observation:

    • Three patients with PCL were analyzed, two with a history of multiple myeloma and one with de novo PCL.
    • Diagnosis required at least 20% plasma cells in peripheral blood.
    • Immunophenotyping revealed plasma cells strongly positive for acid phosphatase and alpha-naphthyl acetate esterase, and negative for periodic acid-Schiff.

    Findings:

    • Patients received varied treatments including polychemotherapy (vincristine, cyclophosphamide, chlorambucil, prednisone), melphalan and prednisone, or total body irradiation.
    • Therapeutic outcomes were poor, with survival not exceeding 3 months in any case.
    • These findings align with the generally dismal prognosis reported for plasma cell leukemia.

    Implications:

    • The study highlights the aggressive nature and poor prognosis of plasma cell leukemia, irrespective of prior myeloma history.
    • Enzyme marker positivity (acid phosphatase, alpha-naphthyl acetate esterase) aids in PCL diagnosis.
    • Further research into novel therapeutic strategies is warranted to improve survival outcomes for PCL patients.
