    Background:

    • Clear-cell acanthoma is a benign skin neoplasm.
    • Unusual locations present diagnostic challenges.

    Observation:

    • A case of clear-cell acanthoma in the para-anal region was examined.
    • Electron microscopy and immunohistochemistry with monoclonal antibodies were employed.

    Findings:

    • Reduced Langerhans cells (2.2%) in affected epidermis compared to normal (4-6%).
    • Disordered keratinization and epidermal differentiation indicated by abnormal monoclonal antibody KL1 and BL7 reactivity.
    • Absence of human papilloma virus involvement.
    • No evidence of eccrine differentiation in tumor cells.
    • Presence of pemphigus antigen in the intercellular space of affected epidermis.
