Background:

Pemphigus vegetans is a rare autoimmune blistering disease.
It is characterized by warty vegetations and erosions, often in intertriginous areas.
Two distinct clinical variants, Neumann and Hallopeau types, are recognized.

Observation:

This study presents two cases of pemphigus vegetans: one Neumann type and one Hallopeau type.
Clinical presentation and disease course were key differentiating factors.
Histologic and immunopathologic findings were similar across both types and resembled pemphigus vulgaris.

Findings:

The Hallopeau type typically presents with a milder clinical course, requiring lower corticosteroid doses and achieving longer remissions.
The Neumann type follows a more aggressive pattern, akin to pemphigus vulgaris, necessitating higher corticosteroid doses and experiencing frequent relapses.
Immunopathologic analysis revealed indistinguishable features between the two types, with IgM staining of eosinophilic microabscesses noted in one case.

Implications:

Understanding the distinct clinical trajectories of Hallopeau and Neumann pemphigus vegetans is crucial for tailored patient management.
While immunopathology is similar, differing clinical courses necessitate individualized treatment strategies, particularly regarding systemic corticosteroid use.
Further research into the pathogenesis of these subtypes may reveal novel therapeutic targets for pemphigus vegetans.
