    Summary

    A diastolic murmur in advanced renal failure patients is often functional, not organic. This murmur disappears with hemodialysis or renal transplantation, indicating a reversible condition.

    Effects of red rest and prazosin in congestive heart failure.

    Background:

    • Patients with advanced renal failure may present with cardiac murmurs.
    • A diastolic decrescendo murmur along the left sternal border was observed in 21 patients.

    Observation:

    • Eight patients had confirmed organic aortic insufficiency.
    • In 13 patients without organic aortic insufficiency, angiocardiography revealed aortic regurgitation in 4 and coronary artery stenosis in 2.
    • Autopsies showed normal aortic ostia in deceased patients.

    Findings:

    • The diastolic murmur in advanced renal failure is likely functional.
    • This functional murmur resolves with renal replacement therapy, including hemodialysis and renal transplantation.

    Implications:

    • The study suggests a non-organic cause for diastolic murmurs in renal failure patients.
    • Clinical management should consider the reversibility of these murmurs with dialysis or transplantation.
    • Further research into the mechanisms of functional murmurs in renal failure is warranted.
