    Propranolol can unmask latent dystrophia myotonica, a genetic muscle disorder. Myotonia symptoms resolved after discontinuing this heart medication, suggesting a drug-induced effect.

    • A 39-year-old male patient with ischaemic heart disease presented with new-onset clinical myotonia.
    • The myotonia symptoms emerged during propranolol therapy.
    • Myotonia resolved upon cessation of propranolol administration.

    Findings:

    • The patient was diagnosed with dystrophia myotonica, which was not clinically apparent prior to propranolol treatment.
    • Propranolol therapy appeared to unmask or exacerbate a previously subclinical dystrophia myotonica.

    Implications:

    • Beta-blockers like propranolol may unmask latent dystrophia myotonica in susceptible individuals.
    • This case highlights the importance of considering drug-induced myotonia in patients with unexplained neuromuscular symptoms.
    • Further investigation into the interaction between propranolol and dystrophia myotonica is warranted.
