    Summary

    Patients with ankylosing spondylitis develop fragile, osteoporotic spines prone to fractures from minor trauma. Diagnosis can be missed without neurological damage, especially in the cervical spine.

    Area of Science:

    Background:

    • Long-standing ankylosing spondylitis (AS) leads to spinal ankylosis and osteoporosis.
    • These spinal changes significantly increase fracture risk, often from minimal trauma.

    Observation:

    • Ankylosed osteoporotic spines in AS patients are susceptible to fractures, frequently in the cervical region.
    • Fractures may be multiple and difficult to visualize radiographically due to spinal deformity.
    • Diagnosis is often delayed or missed unless neurological deficits arise.

    Findings:

    • Cervical spine fractures are common in AS patients.
    • Radiographic challenges exist due to osteoporosis and spinal deformity.
    • Tomography can aid in fracture visualization.
