    Summary

    Saphenous vein patch grafts (SVPG) improve internal carotid artery (ICA) configuration and reduce occlusion after carotid endarterectomy. This study found SVPGs led to better angiographic outcomes and fewer complications compared to conventional methods.

    Background:

    • Carotid endarterectomy is a common procedure to prevent stroke.
    • Conventional techniques may lead to complications such as thrombosis and stenosis.
    • Saphenous vein patch graft (SVPG) offers an alternative reconstruction method.

    Purpose of the Study:

    • To compare the early postoperative outcomes of conventional carotid endarterectomy (CE) with those using a saphenous vein patch graft (SVPG).
    • To evaluate the angiographic findings and complication rates between the two surgical techniques.

    Main Methods:

    • Retrospective analysis of early postoperative intravenous digital subtraction angiograms (IVDSA) in 61 patients (70 conventional CE) and 46 patients (50 SVPG CE).
    • Assessment of internal carotid artery (ICA) origin size and configuration, presence of ballooning, and arteriotomy site stenosis.
    • Documentation of postoperative complications including thrombosis, occlusion, and cerebral infarcts.

    Main Results:

    • SVPG group showed significantly more normal IVDSA findings (p=0.004) and larger, more normal ICA origins.
    • No occlusions occurred in the SVPG group, compared to three in the conventional group (p=0.04).
    • Distal ICA stenosis was significantly less frequent with SVPG (p=0.03).

    Conclusions:

    • Saphenous vein patch graft reconstruction is associated with superior early angiographic results and reduced occlusion rates after carotid endarterectomy.
    • The findings support the routine use of SVPG in carotid endarterectomy to improve outcomes.
