    Background:

    Observation:

    • Two cases of hypertension were observed in patients with unilateral hydronephrosis.
    • Diagnostic evaluations included assessing renin levels in the renal veins.
    • Captopril challenge tests were performed to evaluate the renin-angiotensin-aldosterone system's response.

    Findings:

    • Lateralization of renal vein renins indicated the affected kidney's role in hypertension.
    • Exaggerated hyperreninemia after captopril administration further supported a renin-mediated mechanism.
    • One patient experienced a significant improvement in hypertension following nephrectomy.

    Implications:

    • Unilateral hydronephrosis should be considered in the differential diagnosis of secondary hypertension.
    • Renal vein renin measurements and captopril testing can help identify renin-mediated hypertension.
    • Nephrectomy may be an effective treatment for hypertension caused by unilateral hydronephrosis.
