    Summary

    After-exercise thermography (AET) can identify patients at high risk for deep vein thrombosis (DVT) after surgery. Abnormal AET results correlated with a significantly higher incidence of postoperative DVT in both abdominal and hip surgery patients.

    Background:

    • Postoperative deep vein thrombosis (DVT) is a significant risk following major surgery.
    • Current diagnostic methods for identifying at-risk patients preoperatively are limited.

    Purpose of the Study:

    • To evaluate the efficacy of after-exercise thermography (AET) as a predictive tool for postoperative DVT.
    • To determine if AET can identify patients at higher risk for thromboembolic events.

    Main Methods:

    • After-exercise thermography of the legs was performed on 50 patients before major abdominal surgery and 59 patients before elective hip surgery.
    • Postoperative DVT was diagnosed using the 125I-fibrinogen uptake test or phlebography.
    • Incidence of DVT was analyzed based on normal, unilaterally abnormal, and bilaterally abnormal AET results.

    • Patients with abnormal AET showed a significantly higher incidence of postoperative DVT compared to those with normal AET.
    • In group 1 (abdominal surgery), DVT occurred in 2/19 with normal AET, 12/19 with unilateral AET abnormality, and 11/12 with bilateral AET abnormality.
    • In group 2 (hip surgery), DVT occurred in 9/30 with normal AET, 5/9 with unilateral AET abnormality, and 15/20 with bilateral AET abnormality.

    Conclusions:

    • Preoperative after-exercise thermography is a simple and objective method for identifying patients at high risk of postoperative thromboembolic disease.
    • Abnormal AET findings strongly correlate with an increased likelihood of developing DVT.
    • AET can aid in risk stratification and potentially guide prophylactic strategies for DVT prevention.
