    Background:

    • Fournier's gangrene is a severe necrotizing fasciitis affecting the male genitalia and perineum.
    • This retrospective study analyzes treatment outcomes for severe Fournier's gangrene cases.

    Observation:

    • A 5-year study at King Edward VIII Hospital, Durban, included 43 men with severe Fournier's gangrene.
    • Cases involved the scrotum, perineum, and sometimes the abdominal wall.

    Findings:

    • The overall mortality rate was 18.6% (8 deaths).
    • Mortality was significantly higher in diabetic patients (33%) compared to non-diabetic patients (14.7%).
    • Delayed presentation (over 6 days) was associated with increased mortality.

    Implications:

    • Aggressive surgical debridement and frequent revisions are crucial.
    • Broad-spectrum antibiotics are a key component of treatment.
    • Prompt medical attention is vital to reduce mortality in Fournier's gangrene.
