Endophthalmitis caused by Propionibacterium acnes.

Summary

A rare case of endophthalmitis caused by Propionibacterium acnes, a common conjunctival bacterium, was successfully treated. Prompt intervention with vitrectomy, antibiotics, and anti-inflammatory medication led to rapid visual recovery.

Background:

Endophthalmitis is a severe intraocular infection often leading to vision loss.
Propionibacterium acnes is a commensal bacterium of the conjunctiva, rarely implicated in endophthalmitis.

Observation:

A patient presented with endophthalmitis six weeks post-ocular injury.
Cultures revealed the presence of Propionibacterium acnes.

Findings:

The patient underwent surgical intervention (vitrectomy and lensectomy).
Treatment included intravitreal antibiotics, systemic cephalothin, and prednisone.
Rapid clinical and visual improvement was observed post-treatment.

Implications:

This case highlights the potential for P. acnes to cause endophthalmitis, even after minor trauma.
Effective management involves prompt surgical and antimicrobial therapy.
Early diagnosis and treatment are crucial for favorable visual outcomes in P. acnes endophthalmitis.
