    Background:

    • Mechanical urticaria is a physical urticaria characterized by wheal and flare reactions upon skin contact with mechanical stimuli.
    • The underlying causative agents and mechanisms of mechanical urticaria remain incompletely understood.

    Observation:

    • Sera from patients with symptomatic mechanical urticaria induced immediate weal and flare reactions upon intracutaneous injection into recipients.
    • Pretreatment of skin test sites with Compound 48/80 significantly reduced or abolished these induced reactions.

    Findings:

    • A weal-producing substance was isolated and co-eluted with Immunoglobulin M (IgM) during Sephadex G-200 gel filtration.
    • This substance demonstrated heat stability (56°C for 4 hours) but lability upon treatment with 2-mercaptoethanol.
    • Immunoadsorption using anti-IgM serum confirmed the identity of the weal-producing substance as IgM.

    Implications:

    • This study identifies Immunoglobulin M (IgM) as a key mediator in the pathogenesis of mechanical urticaria.
    • Understanding IgM's role offers potential targets for novel diagnostic and therapeutic strategies for physical urticaria.
    • Further research into IgM-mediated pathways could elucidate mechanisms in other hypersensitivity reactions.
