    Background:

    • Beta cell dysfunction is central to diabetes mellitus.
    • Metabolic derangement in diabetes can impair insulin secretion.
    • Understanding beta cell response to metabolic control is crucial.

    Purpose of the Study:

    • To investigate the effect of metabolic control on beta cell function in diabetic patients.
    • To quantify changes in pro-insulin and insulin levels with improved metabolic state.

    Main Methods:

    • Plasma pro-insulin and insulin levels were measured using gel-filtration.
    • Measurements were taken in three diabetic patients at two time points: before and after therapy.
    • Patients were assessed in states of severe metabolic derangement and after improvement.

    Main Results:

    • Pro-insulin concentrations remained unchanged (30 pM) with or without therapy.
    • Fasting insulin levels doubled (from 22 pM) with treatment.
