    A new noninvasive method estimates carbon dioxide (CO2) response in infants by measuring oxygen (O2) levels. This technique accurately assesses ventilatory response to CO2 in uncooperative infants.

    Background:

    • Assessing ventilatory response to carbon dioxide (CO2) is crucial for diagnosing respiratory control disorders.
    • Current methods often require patient cooperation, limiting their use in infants.

    Purpose of the Study:

    • To evaluate a novel noninvasive method for estimating ventilatory response to CO2 in uncooperative infants.
    • To compare the efficacy of three indirect methods against the standard steady-state method.

    Main Methods:

    • Compared changes in inspired minus alveolar partial pressure of oxygen (delta IAPO2), inspired minus alveolar partial pressure of carbon dioxide (delta AIPCO2), and transcutaneous partial pressure of oxygen (TcPO2).
    • Utilized the standard steady-state method as the reference for ventilatory response to CO2.
    • Conducted 21 comparisons in 7 preterm infants and 1 older child with central hypoventilation syndrome.

    Main Results:

    • Changes in delta IAPO2 showed the best correlation with minute ventilation (r = 0.83, p < 0.001).
    • Changes in delta AIPCO2 demonstrated a weaker correlation (r = 0.66, p < 0.001).
    • No significant correlation was found between changes in TcPO2 and minute ventilation.

    Conclusions:

    • Changes in inspired minus alveolar PO2 (delta IAPO2) provide a reliable, noninvasive method for estimating CO2 ventilatory response in uncooperative infants.
    • This method offers a valuable alternative to traditional techniques, particularly in challenging pediatric populations.
    • Delta AIPCO2 and TcPO2 are less suitable for estimating ventilatory response to CO2 in this context.
