    Summary

    Clonazepam is more effective than lithium for acute mania treatment, reducing the need for antipsychotics like haloperidol. This may lower the risk of tardive dyskinesia in patients with mania.

    Background:

    • Acute mania is a severe neuropsychiatric condition requiring effective treatment.
    • Lithium carbonate is a standard treatment, but its efficacy and side effect profile can vary.
    • Reducing the need for adjunctive antipsychotics is desirable to minimize long-term risks.

    Purpose of the Study:

    • To compare the efficacy of clonazepam and lithium carbonate in treating acute manic episodes.
    • To assess the requirement for adjunctive haloperidol during treatment with clonazepam versus lithium.

    Main Methods:

    • A double-blind, randomized, cross-over study involving 12 newly admitted acutely manic patients.
    • Patients received 10 days of clonazepam followed by 10 days of lithium, or vice versa.
    • Efficacy was assessed by reduction in manic symptoms and need for PRN haloperidol.
