    Area of Science:

    • Orthopedics
    • Pharmacology
    • Radiology

    Background:

    • Paget's disease of bone and myositis ossificans are bone conditions.
    • Disodium etidronate (EHDP) is a bisphosphonate medication used to treat bone diseases.

    Observation:

    • Four patients experienced localized foot pain during EHDP treatment.
    • Pain was associated with tenderness and mild swelling, impacting mobility.
    • Radiographs were normal, but bone scans indicated increased isotope uptake at painful sites.

    Findings:

    • The observed symptoms were consistent with diphosphonate-induced stress fractures.
    • Pain persisted for weeks, causing significant walking difficulties.

    Implications:

    • This suggests EHDP should be administered at low doses for limited durations.
    • Careful monitoring is essential to prevent adverse effects like stress fractures.
