Colostomy closure. Morbidity reduction employing a semi-standardized protocol.

R K Salley, R M Bucher, C B Rodning

Diseases of the Colon and Rectum
    |May 1, 1983
    PubMed
    Summary

Closing colostomies sooner than 8.5 weeks increases major and septic complications. Simple transverse closure is as safe as resection and anastomosis, and primary wound closure is recommended for colostomy closure safety.

    Area of Science:

    • Colorectal Surgery
    • Surgical Outcomes
    • Patient Safety

    Background:

    • Colostomy closure is a common surgical procedure.
    • Standardized protocols can improve patient outcomes.
    • Optimizing the timing and technique of colostomy closure is crucial.

    Purpose of the Study:

    • To evaluate a semi-standardized protocol for colostomy closure.
    • To identify factors influencing morbidity and complications.
    • To assess the safety of different closure techniques.

    Main Methods:

    • Retrospective analysis of 166 consecutive colostomy closure cases (1974-1981).
    • Evaluation of complication rates based on closure interval and surgical technique.
    • Statistical analysis to determine significant associations (P <= 0.001).

    Related Experiment Videos

    Main Results:

    • Overall morbidity rate was 2.4% (17/166).
    • Colostomies closed <8.5 weeks had significantly higher major and septic complications (P <= 0.001).
    • Simple transverse closure showed no increased morbidity compared to resection/anastomosis (P <= 0.1).
    • Primary wound closure (with or without drain) had a low infection rate (1.2%).

    Conclusions:

    • Closing colostomies at intervals <8.5 weeks is associated with increased risk.
    • Simple transverse colostomy closure is a safe alternative to resection and anastomosis.
    • Primary wound closure is a safe and desirable method for colostomy closure.
