    Adverse effects from transdermal nitroglycerin are rare. A patient experienced a skin reaction to the Transderm-Nitro system, likely due to the delivery patch itself, not the medication.

    Background:

    • Transdermal nitroglycerin systems are commonly prescribed for angina pectoris.

    Observation:

    • A 63-year-old male patient with multiple comorbidities developed a localized cutaneous reaction after prolonged use of Transderm-Nitro-5 patches.
    • The reaction manifested as macular erythematous areas at the exact application sites of the nitroglycerin patches.

    Findings:

    • A subsequent challenge with a placebo Transderm-Nitro patch in the same patient reproduced a similar-appearing skin reaction.
    • These findings suggest the cutaneous reaction is attributable to the transdermal delivery system components rather than the active pharmaceutical ingredient, nitroglycerin.

    Implications:

    • This case highlights a potential, albeit rare, adverse reaction to the excipients or materials within transdermal drug delivery systems.
    • Further investigation into the specific components of transdermal patches may be warranted to identify causative agents for such reactions.
