    • The patient was diagnosed with tuberculous meningitis.
    • The focal encephalitis represented an uncommon manifestation of TBM.
    • This case underscores the varied clinical and radiological presentations of TBM.

    Implications:

    • Tuberculous meningitis should be considered in the differential diagnosis of space-occupying brain lesions, even with atypical imaging findings.
    • High clinical suspicion is crucial for timely diagnosis and treatment of TBM.
    • Awareness of unusual neurological syndromes associated with TBM can improve patient outcomes.
