Serum digoxin levels--a cost/benefit analysis.

D J Skene, A Modipane, P H Joubert

South African Medical Journal = Suid-Afrikaanse Tydskrif Vir Geneeskunde
    |January 7, 1984

    Summary

Serum digoxin level testing is often clinically useless due to improper blood sample timing and missing information. Proper training in pharmacokinetic principles is needed for accurate digoxin level interpretation.

    Area of Science:

    • Clinical Chemistry
    • Pharmacokinetics
    • Laboratory Medicine

    Background:

    • Serum digoxin levels are crucial for therapeutic drug monitoring.
    • Inaccurate sampling compromises the clinical utility of digoxin assays.

    Purpose of the Study:

    • To evaluate the clinical usefulness of serum digoxin level determinations.
    • To identify factors contributing to the invalidity of test results.

    Main Methods:

    • Retrospective analysis of 132 serum digoxin level tests.
    • Review of associated sampling information, including timing and completeness.

    Main Results:

    • Two-thirds of serum digoxin samples were clinically unusable.
    • Half of unusable samples lacked essential sampling information.
    • The other half of unusable samples had incorrect collection times.

    Conclusions:

    • A significant proportion of serum digoxin tests provide no clinical value.
    • Inadequate sampling information and incorrect timing are primary reasons for unusable results.
    • Medical practitioner education in pharmacokinetics is essential for improving the quality of digoxin testing.
