Carcinoid tumour secreting dopa.

B K Sharma, B J Smits, R Robinson

British Medical Journal (Clinical Research Ed.)
    |April 14, 1984
    PubMed
    Summary

A liver carcinoid tumor caused high levels of dopa metabolites due to an enzyme defect. Surgical removal significantly reduced these metabolites, confirming the tumor as the source.

    The Lancet. Oncology·2021

    Area of Science:

    • Gastroenterology
    • Endocrinology
    • Oncology

    Background:

    • Investigating the source of an abdominal mass in a middle-aged woman.
    • Assessing unusual urinary metabolite levels.

    Observation:

    • Patient presented with an abdominal mass and significantly elevated urinary dopa (3-4-dihydroxyphenylalanine) metabolites.
    • Surgical exploration revealed a large tumor occupying the left lobe of the liver.
    • Histological examination identified the tumor as a metastatic carcinoid.

    Findings:

    • Post-operative urine analysis showed a substantial decrease in dopa metabolite excretion.
    • The metastatic carcinoid tumor was identified as the source of the elevated metabolites.
    • The tumor exhibited an enzyme defect, impairing its ability to decarboxylate dopa.

    Implications:

    • This case provides further evidence supporting a neural origin for the gut's endocrine system.
    • Highlights the diagnostic utility of metabolite analysis in identifying rare tumor types.
    • Demonstrates a specific metabolic anomaly in metastatic carcinoid tumors.
