    The Five Hospital Homebound Elderly Program (FHHEP) reduced nursing home admissions and improved well-being for chronically impaired seniors. While costs were higher, quality of life increased, demonstrating a valuable model for comprehensive home care.

    Background:

    • The Five Hospital Homebound Elderly Program (FHHEP) is a model for long-term, comprehensive, coordinated home care.
    • Evaluating such programs is crucial for understanding their impact on elderly populations with chronic impairments.

    Purpose of the Study:

    • To assess the 9-month outcomes of the FHHEP model program.
    • To evaluate mortality, functional status, hospitalization, and institutionalization rates among participants.

    Main Methods:

    • A quasi-experimental, pre-posttest design with a nonequivalent control group was employed.
    • 122 experimental clients and 123 control clients (receiving OAA Title III-c meals) were assessed using the Duke/OARS Multi-dimensional Functional Assessment Questionnaire.
    • Multivariate analysis controlled for pretest differences; service utilization data were also collected.

    Main Results:

    • Significant reductions in nursing home admissions (16 vs. 28) and nursing home days were observed in the FHHEP group.
    • Experimental clients reported improved sense of physical health well-being and fewer unmet community service needs.
    • A decrease in physical activities of daily living (PADL) functioning was noted, alongside equal mortality and hospitalization rates between groups.

    Conclusions:

    • The FHHEP model effectively reduced nursing home utilization and improved perceived health and service access for the elderly.
    • Despite a 19% increase in per-capita costs, the program enhanced quality of life, suggesting a favorable cost-benefit trade-off.
    • Further long-term assessment is ongoing to fully evaluate the program's impact.
