    Summary

    Hearing preservation is achievable in small acoustic neurinomas using a combined surgical approach. This technique offers good outcomes for patients with tumors less than 2 cm, minimizing hearing loss.

    Background:

    • Acoustic neurinomas are benign tumors that can affect hearing and facial nerve function.
    • Surgical intervention is a primary treatment modality for acoustic neurinomas.
    • Preserving hearing and facial nerve function are critical goals in acoustic neurinoma surgery.

    Observation:

    • The study evaluated a combined suboccipital/transmeatal surgical approach in 21 patients with acoustic neurinomas between 1974 and 1982.
    • Tumor size and brainstem involvement were key factors influencing surgical outcomes.
    • Patients were operated on in a seated position.

    Findings:

    • For 12 patients with tumors smaller than 2 cm, hearing was preserved at or near preoperative levels in seven, with improvement in one.
    • Tumors larger than 2 cm or those involving the brainstem resulted in lower rates of hearing preservation.
    • No tumor recurrences or hearing diminution were observed on CT scans up to 7 years post-surgery.
    • Seventh cranial nerve function was preserved in all patients, with mild complications.

    Implications:

    • The combined suboccipital/transmeatal approach is effective for hearing preservation in select acoustic neurinoma patients.
    • Early surgical intervention for smaller tumors maximizes the chances of hearing preservation.
    • This surgical technique demonstrates a favorable safety profile with preserved facial nerve function.
