    On call. As a physician, I find Harvard Men's Health Watch very helpful for my patients and as a 63-year-old man, I find it interesting and helpful for myself. But I'm puzzled by your November 2001 article, which recommends two drinks a day as the safe maximum. I remember reading an editorial in The Lancet a few years ago that recommended up to 25 drinks a week, or three a day. Which dose of alcohol is correct?

    On call. I m a healthy 44-year-old man. I ve always been heavy, but eight months ago I started a diet and exercise program. I ve lost 26 pounds and I feel great. Now that I'm thinner, I thought it would be a good time to buy more life insurance, but a friend warned me that if I report the weight loss my premiums might go up.     Background:

    • Bacterial meningitis is typically a rapidly progressive and life-threatening condition.
    • Neisseria meningitidis is a common cause of bacterial meningitis.

    Observation:

    • Three patients with meningococcal meningitis presented with an indolent, slow-progressing clinical course.
    • Symptoms persisted for 2 days to 4 weeks before hospitalization.
    • Cerebrospinal fluid cultures confirmed Neisseria meningitidis infection days before antibiotic treatment, yet patients remained stable.

    Findings:

    • All three patients recovered fully following penicillin therapy.
    • The indolent course was characterized by benign cerebrospinal fluid findings and intact neurological status.
    • This contrasts with the usual aggressive presentation of meningococcal meningitis.

    Implications:

    • Recognizing atypical, slow-progressing meningococcal meningitis is crucial for earlier diagnosis.
    • Further immunologic studies are needed to understand the pathogenesis of this indolent syndrome.
    • Increased awareness can improve patient outcomes by facilitating timely intervention.
