    Background:

    • Erectile impotence is a frequent complaint among alcoholics, yet its underlying mechanisms remain unclear.
    • Alcohol abuse is associated with various health issues, including sexual dysfunction.

    Purpose of the Study:

    • To investigate the mechanisms of erectile impotence in alcoholic patients.
    • To differentiate between psychogenic and organic causes of impotence in this population.

    Main Methods:

    • Studied nocturnal penile erections in 13 alcoholic men complaining of impotence.
    • Assessed plasma concentrations of follicle-stimulating hormone (FSH) and luteinizing hormone (LH).
    • Evaluated for neurological damage, including to the parasympathetic nervous system.

    Main Results:

    • Seven patients had normal nocturnal erections, suggesting psychogenic impotence.
    • Six patients exhibited diminished or absent nocturnal erections.
    • The latter group showed elevated FSH and LH levels (with one exception of elevated FSH only) and more signs of neurological damage.

    Conclusions:

    • Impaired nocturnal erections in alcoholic men may indicate organic causes.
    • Hormonal and neurological assessments during sleep studies can help distinguish organic from psychogenic erectile impotence in alcoholics.
