    • Alpha heavy-chain disease (alpha HCD) is a rare lymphoproliferative disorder characterized by the production of abnormal immunoglobulin heavy chains.
    • Typically, alpha HCD primarily affects the gastrointestinal tract, leading to malabsorption and abdominal symptoms.

    Observation:

    • This case report details an unusual presentation of alpha HCD in a patient with a goiter.
    • The goiter was identified as an extramedullary plasmacytoma, a tumor arising from plasma cells outside the bone marrow.

    Findings:

    • Diagnosis of alpha HCD was confirmed using standard laboratory tests and high-resolution two-dimensional gel electrophoresis.
    • Analysis revealed an abnormal alpha chain fragment with a molecular weight of 38,000 Daltons.
    • This fragment exhibited seven distinct charge forms within an isoelectric point range of 5.2-5.6.

    • The thyroidal involvement in this case is atypical for alpha HCD, highlighting the diverse clinical manifestations of the disease.
